Your Ferritin Result, Decoded: What Each Range Means When Inflammation Is in the Mix

Can inflammation raise ferritin? Yes, and that is why the same number can mean low iron, plenty of iron or a body that is busy fighting something. Here is what each range means, and the one extra test that sorts it out.

The short answer

Yes. Ferritin stores iron, but it also rises with inflammation and infection. A low ferritin almost always means low iron stores. A high one often does not mean too much iron, and most adults with high ferritin turn out not to have true iron overload at all. Transferrin saturation helps sort it out.

This article is part of our series on [how to test for inflammation](/articles/how-to-test-for-inflammation).

The numbers

Labs report ferritin in micrograms per liter (µg/L) or nanograms per milliliter (ng/mL). The two units give the same number. A liter holds 1,000 milliliters, and a microgram is 1,000 nanograms, so the thousands cancel out. 30 µg/L is 30 ng/mL.

Your ferritinWhat it usually points toSource
Under 15 µg/LDepleted iron stores, the survey cutoff for women 15 to 49
Under 30 µg/LLow iron in people seeing a doctor (79% caught, 2% wrongly flagged)
30 to 200 µg/LA gray zone: studies proposed low-iron cutoffs from 12 to 200
Over 200 (women) or 300 (men and postmenopausal women)Possible iron overload, but only with high transferrin saturation (over 45% or 50%)
Over 500 µg/LIn one hospital: infection most often, then tumors, liver and kidney problems
Over 10,000 µg/LA flag for HLH, a rare and dangerous immune overreaction

Here is what that 30 µg/L line means in people. This is our arithmetic, not the review's. The test caught 79% of low iron. So out of every 100 people who truly lack iron, about 21 would get a ferritin above 30 and look fine. It was 98% specific, so out of every 100 people with enough iron, about 2 would be wrongly flagged.

So a low number is a strong signal, but a normal one can still hide a problem.

Can inflammation raise ferritin?

It can, and it does so on purpose. During an infection, higher ferritin is part of the body's defense. It keeps iron away from bacteria so they cannot use it to grow. Doctors call ferritin an acute-phase reactant: a blood protein that climbs with inflammation. Very high levels can be a sign that inflammation is out of control.

A low ferritin is a strong signal, but a normal one can still hide low iron. From a 2021 meta-analysis in Cochrane Database Syst Rev

That is the catch. Ferritin is supposed to show your iron stores, yet it also goes up with inflammation and infection. When you are ill, a normal-looking ferritin may sit on top of empty iron stores.

The largest look at this pooled 8 surveys of 24,844 women aged 15 to 49. CRP (C-reactive protein) is a standard blood marker of inflammation. As CRP went up, fewer women had a ferritin that said "low iron." It was 29% in the lowest CRP tenth and 6% in the highest. That is a 23-point gap. When the researchers corrected for inflammation, the share with low iron stores rose by 2 to 8 points.

What moves the number

Inflammation and infection. These push ferritin up. In 1,394 hospital patients with ferritin over 500, infection outside HIV was the most frequent reason.

Your liver. Fatty liver disease is a common reason. A 2026 meta-analysis pooled 16 studies of people with metabolic fatty liver disease. About 1 in 4 had high ferritin (26.36%). Among people with high ferritin, fatty liver and alcohol-related liver disease are more common than inherited iron overload.

Metabolism and alcohol. Heavy drinking and metabolic syndrome are on the short list of common reasons. Neither one loads the liver with much iron.

Several things at once. More than 40% of people with high ferritin have more than one reason behind it, and that is the kind of overlap that makes a single number so hard to read. In the hospital study, 41% did, and the more of these conditions a patient had, the higher ferritin tended to climb.

Some medicines. A 2026 review says one diabetes drug class, the SGLT2 inhibitors, changes how ferritin should be read.

When a number should worry you

Low ferritin. Low levels invariably indicate reduced iron stores. Under 30 is worth a conversation, even if you feel fine.

High ferritin with high transferrin saturation. Transferrin saturation (TSAT) shows how full your iron-carrying protein is. If it is over 50%, inherited iron overload (hemochromatosis) moves to the top of the list. A European guideline uses two pairs of numbers for people with the main gene change. For women: TSAT over 45% and ferritin over 200. For men and women past menopause: TSAT over 50% and ferritin over 300.

Very high or still rising ferritin. If no one can find the reason, or ferritin keeps going up, the main next step is a liver MRI to measure iron. In hemochromatosis, a ferritin under 1,000 at diagnosis means a low risk of serious liver scarring.

High ferritin with normal iron. This is the common pattern. It usually points to inflammation, liver or kidney disease, cancer or metabolic syndrome, not iron overload. It still needs an explanation. In fatty liver, high ferritin was linked to about double the risk of liver problems later (hazard ratio 2.02).

Common questions

Why is my ferritin high but my iron normal?

Usually, something other than iron is raising it. Inflammation, liver or kidney disease, cancer and metabolic syndrome can all lift ferritin. Fatty liver is a big one. If your transferrin saturation is normal rather than over 50%, inherited iron overload drops down the list.

Is ferritin an inflammatory marker?

Partly. It stores iron, and it also rises with inflammation. So one ferritin result cannot tell you which job it is doing. Doctors pair it with TSAT, CRP and sometimes newer blood markers such as the soluble transferrin receptor.
